The Diabetes Education Program will help people living with diabetes manage their condition, and optimize their quality of health. We will provide diabetes information, referrals, and access to community resources. The Diabetes Education Program is an inter-disciplinary team that will provide diabetes education through Group Classes and Individual Counselling to adults living with Type 2 Diabetes or Pre-diabetes (also called “impaired glucose tolerance” or “impaired fasting glucose”). Additional programs and services will be developed to respond to the needs of the community. Click here to download a copy of the Diabetes Education Program brochure.
