Text Box: A recent report by the Institute of Clinical Evaluative Sciences (ICES) points to extremely high rates of diabetes in the Jane-Finch Community. To help those living in this community to manage their diabetes, Black Creek Community Health Centre is offering a Diabetes Education Program.
 
Goal: To help people living with diabetes manage their condition and improve their quality of health.
 
Team: The Diabetes Education Team includes Diabetes Nurse Educators, a Registered Dietitian, Community Health Worker and a Coordinator.

The Diabetes Education Program is a community-based program that is committed to working with community members and organizations.
 
Focus groups are being organized at various times and locations.  These groups will consist of individuals living in the Jane-Finch community, as well as representatives from local organizations. These focus groups will assist the Diabetes Education Team in the planning of diabetes programs and services.
