Staff Roles and Responsibilities

 

Administrative Team 

 

Executive Director

The Executive Director provides overall leadership and management to the organization and interprets and implements Board policies and strategic directions. 

 

Confidential Executive Assistant

The Confidential Administrative Assistant provides administrative support to the Executive Director and maintains various offices systems.

 

 Data Management Coordinator

The Data Management Co-ordinator co-ordinates and provides data support and data management functions as directed.

 

Financial Manager

The Manager of Finance and Administration co-ordinates the Centre’s financial management, human resources and various office systems.

 

Office Manager

The Office Manager is responsible for organizing and coordinating office operations and procedures in order to ensure organizational effectiveness and efficiency and supervising office staff and maintaining office records. 

 

Medical Secretary

The Medical Secretary operates and maintains appointment, chart management and third party billing systems, and provides clerical, administrative support and reception functions.

 

Receptionist/Secretary

The Receptionist/Secretary provides reception, clerical and administrative support functions and operates and maintains appointment, chart and document management systems.

  

Clinical Team

 

Dietitian

 The Dietitian provides nutritional care functions and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community to promote better nutrition.

  

Chiropodist

The Chiropodist provides foot care functions and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community to promote better foot management.

  

Physician

The Physician provides evaluation and treatment of medical problem, identification and advice on risk factors for disease.  Our Physicians provide high quality medical care to people of all age groups.  They treat illness and help you to learn to keep well and feel healthier. 

 

Nurse Practitioner

The NP’s are specialized nurses whose focus is on holistic health care, health promotion, disease prevention and community health development.  They provide check-ups and vaccines for children, teen and adults. 

  

Social Worker/Therapist

 The Social Worker/Therapist provides counselling/therapy and other therapeutic functions.  The Social Worker/Therapist is involved with group work, individual, couple and/or family counseling as well as referrals to other resources. 

  

Diabetes Education Team

 

Diabetes Program Manager

The Diabetes Program Manager oversees the successful operation of our community Diabetes network. H/she is directly responsible for providing support and leadership to a team of Registered Nurses, Dietitian and Community Health Worker.

 

Diabetes Nurse Educator

The nurse educator is responsible for supporting the development and implementation of the Diabetes Education Program.  The Diabetes Nurse Educator is involved in assessment, group education sessions and individual counseling to those with pre-diabetes and Type 2 diabetes.  

 

Diabetes Dietitian

The Diabetes Dietitian provides information around the prevention, care and management of diabetes. The Dietitian is responsible for the development, implementation, monitoring and evaluation of the Diabetes Education Program.  The Dietitian provides group and individual counseling to clients with pre-diabetes or type 2 diabetes.

  

Diabetes Community Health Worker

The Community Health Worker assists in developing diabetes and health promotion activities. In addition, the CHW leads, assists and/or supports action on community health issues.

Community Health Program Manager

The Community Health Programs Manager co-ordinates and supervises the development, implementation, monitoring and evaluation of programs for adults and seniors, as well as community health promotion program(s), or service(s).

 

Youth Program Manager

The Youth Program Manager co-ordinates and supervises the development, implementation, monitoring and evaluation of community programs and services for youth and their families.

  

Community Health Worker

The Community Health Worker (CHW) leads, assists and/or supports action on community health issues. The CHWs work with members of the community and organizations to identify, amplify and solve health issues through program development, public education, outreach, advocacy and community action.  

  

Early Years

 

Program Manger

The Program Manager co-ordinates and supervises the development, implementation, monitoring and evaluation of the Early Years Program.

  

Lactation Consultant

The Lactation Consultant provides prenatal and postnatal breastfeeding education/ counseling and facilitates a breastfeeding support groups.  This person is responsible for one-on-one assistance and support to group programs.

 

Program Educator

The Early Years Program Educator takes leadership in the development, implementation, operation and evaluation of Early Years services and activities. This person promotes the development of social support networks among parent, caregivers and children.

 

Social Worker/Therapist

The Social Worker/therapist provides individual and/or couples therapy and support to women who are suffering from postpartum depression or who are at risk.   The Social Worker/therapist facilitates a postpartum depression group.
