Black Creek Community Health Centre is pleased to announce an expansion to its Inter-Professional Care Team to serve residents of North York West and North York Central. An Inter-professional care team is made up of different healthcare professionals who offer a larger range of help and are located in one location or closer to home.

What services are part of the BCCHC's Inter-Professional Care Team? (Description of PACT services)

  • Chiropody Assessment and Screening - Foot Care
  • Chronic Obstructive Pulmonary Disease (COPD) - Lung Health Program (Spirometry, Smoking cessation, exercise)
  • Community Registered Dietitian
  • Community Supports and Referrals
  • Diabetes Education and Self Management
  • Diabetic Retinopathy Screening - Teleopthalmology Program (TOP)
  • Harm Reduction - Supports and Referrals
  • International Board Certified Lactation Consultant - Breastfeeding Support
  • Physiotherapy (Non-MVA)
  • Registered Kinesiologist
  • Social Work/Therapist

Who can access these services?

Through this expansion, BCCHC will offer access to these different health services for patients of Physicians or Nurse Practitioners who do not work in a larger inter-professional team. These doctors will now have a centralized team where they can refer their patients.

Anyone using the service(s) will still be patients with their existing primary care provider, while accessing the complimentary services from BCCHC. With the consent of the patient, BCCHC will provide timely and confidential updates to referring providers to keep a clear flow of communication while providing the full range of health care.

Is there a cost?

Services that are typically paid for will be offered for free to those without extended health benefits when referred by their Family Physician  or Nurse Practitioner.

What other services are offered?

Black Creek Community Health Centre also provides systems navigation for those who are struggling to connect to the services they need. For instance, the Systems Navigator can help someone who needs assistance completing forms for passports, ODSP, immigration, social insurance, and pension benefits.

We provide translation services to help clients get the best possible care and information.
