COVID-19 Support Line

Call the Black Creek COVID-19 Support Line at 416-830-3229 any day from 10 am to 8 pm to learn how you & your family can access:

Self-isolation Support

If you test positive for COVID-19, or if you have been exposed to someone who has tested positive for COVID-19, you must self-isolate at home for 14 days. Call us at 416-830-3229 any day from 10 am to 8 pm to discuss options, including community isolation centres, how we can support you to isolate at home or how to access voluntary isolation centres.

Voluntary Isolation Centres

To keep everyone else at home safe and to prevent spreading infection to family members, coworkers, and others in the community, self-isolation is the best solution, but it’s not always possible.

Since it’s often difficult to self-isolate at home, Voluntary isolation centres offer free, comfortable, safe and secure accommodation. Healthy meals and snacks are provided throughout the isolation period.

You deserve reliable information. You can count on Black Creek Community Health Centre to provide you with accurate information to make the best decisions for yourself and your family every day about how you live, work and play.

You deserve access to reliable information and care in your community so make the best decisions to protect yourself and your family.

You can get a COVID-19 appointment to get tested if you:

  • have symptoms of COVID-19
  • have been exposed to someone who has a positive COVID-19 test
  • live or work where there has been a COVID-19 outbreak
  • are eligible for testing as part of Ontario’s targeted testing initiative

You are eligible for testing as part of Ontario’s targeted testing initiative if you are:

  • resident or worker in a long-term care home
  • visit a long-term care home
  • resident or worker in a homeless shelter
  • international student
  • farm worker
  • member of an Indigenous community
  • specific populations (asymptomatic patients with cancer; patients having surgery etc.)
  • children and teenagers as directed based on the Ministry of Health and Long Term Care’s COVID-19 School & Care Screening

Vaccination protects us against the COVID-19 virus by enhancing our natural ability to fight infection. Vaccines help stop spread of the virus so we can all get back to our normal routines. More than 500,000 people in Toronto have already been vaccinated. One by one, we can get this done, together.
