Health Coaching to Wellness

 

 

 

 

Health Coaching to Wellness  is an innovative partnership between Black Creek Community Health Centre and NexJ Health. 

The Program is part of the Type 2 Diabetes Prevention Challenge, an opportunity to bring together innovators and community to codesign approaches to Type 2 Diabetes across Canada.  

The Health Coaching to Wellness Program includes the following activities:

  • Recruit and train community members to be lay Health Coaches so that they can use their skills and experience as  to help connect people with the services and supports they need to reduce their risk of developing diabetes.
  • Provide FREE health coaching to community members to help live an active, health life and reduce their risk for developing Diabetes.

 

Who are Community Health Coaches?

Health Coaches are people from the community with real life experiences and real motivation to help YOU achieve success! Your Community Health Coach will:

  • customize a unique plan that works for YOU
  • help you set SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goals.
  • keep you motivated to reach your goals and make them stick
  • share tools and resources to support wellness – mind, body and soul.

Bringing Health Coaching to Communities across Ontario!

May 20, 2026: Black Creek Community Health Centre and Kinvia were named as the Grand Prize Winners, receiving $1.25 million each, from the Public Health Agency of Canada, for the Canada’s Diabetes Prevention Challenge.

On behalf of the Minister of Health, the Honourable Sonia Sidhu, MP, Brampton South, announced two grand prize winners: Black Creek Community Health Centre and Kinvia, who are recognized for the long-term potential of their diabetes prevention programs to support people at risk of developing type 2 diabetes.

As part of the next phase of the program, Black Creek Community Health Centre will be bringing Health Coaching to communities across Ontario.

As a Community Ambassador with Black Creek CHC, Jenan is committed to her community and eager to support clients to achieve success.

“As a personal trainer, I remind clients health goals are just as important as physical and fitness goals.”

Since her father is living with Diabetes, this is Nadia’s way of supporting her father and others like him.

Nayefa

“Good information helps people make good lifestyle choices.”

Since Diabetes runs in her family, Nayefa is encouraged that members of her community have equal access to programs that can prevent the disease.
