At Black Creek CHC, the care team consists of physicians, nurse practitioners, nurses, chiropodists, social workers, respiratory therapists, nutritionists, health promotion and harm reduction providers, among other allied healthcare providers. This collaborative, holistic approach considers practical factors to support health, wellness, and wellbeing.
