The People Accessing Care Teams (PACT) program connects clients with a holistic approach to comprehensive care in the community.

At Black Creek CHC, the interdisciplinary care team engages individuals, families, and organizations to deliver comprehensive health services and programs that enhance community health and wellbeing. 

The care team at Black Creek CHC works with community healthcare providers as part of an interprofessional team to serve the growing and diverse population in Northwest Toronto.

Services offered through PACT include:

  • Chiropody (foot care) – assessment and screening
  • Diabetes education and self-management
  • Dietitian
  • Harm reduction – support and referrals
  • Kinesiology
  • Lactation consultant – breastfeeding support
  • Lung health – spirometry, smoking cessation and exercise
  • Mental Health Counselling
  • Midwifery
  • Physiotherapy
  • Sexual health
 

If you’re a healthcare provider in Toronto’s Northwest or Central North York, join Black Creek CHC’s inter-professional care team, which offers your patient access to a multi-disciplinary team.

With patient consent, a confidential update can be shared back to you, as your patient’s healthcare provider, to maintain a continuum of care.

Who can access PACT?

Through PACT, clients with a referral from a community nurse practitioner or physician, can receive care at Black Creek CHC, at no cost.
