Who qualifies for Primary Care Services?
You may qualify for our primary care services if any of the following apply to you:
- You live within our service area –> Refer to Map
- You identify as Francophone
- You identify as Indigenous — First Nations, Inuit, or Métis

If you are eligible, a member of our team will contact you regarding next steps.
We make every effort to register clients. If we are unable to accept you as a client, we will do our best to connect you with other
health and community resources who can help you.
What Happens Next?
1. Submit your application using the secure form below.
2. Our intake team will review your application to determine eligibility.
3. If you are eligible, you will be contacted to schedule an intake appointment.
4. Attend your intake appointment, where we will learn more about your health needs, and any supports or services that may be helpful to you.
5. Following your intake appointment, you will be scheduled with a doctor or nurse practitioner for your first appointment.

The information you provide in this application will be kept private and secure.
Your information will only be used by our intake team to:
- Determine if you are eligible for our services
- Contact you regarding your application
- Connect you with services to help meet your health and wellbeing needs.
We will not share your personal information without your consent unless required by law.
