Black Creek CHC’s physicians and nurse practitioners assess, treat, and educate to prevent illness, manage disease, and support chronic disease management. Same day and next-day appointments are available as required.
After-hours Service
When our centre is closed, there is a doctor on-call who can speak to you on the phone about your immediate medical concern.

You must be registered as a client of Black Creek Community Health Centre to access primary care services.
If you don’t have a primary care provider, you will need to fill out a client intake form.
