Black Creek Community Health Centre is joining the province-wide campaign led by the Alliance for Healthier Communities under the theme “Standing Together.”

    • With more investment, Community Health Centres can expand services, meet rising community needs, and achieve even better health outcomes.

  • Commitment to Equity, Innovation & Inclusivity

    • Community Health Centres  use data-driven, evidence-based approaches, prioritize health equity, lead in social prescribing, and provide culturally safe, inclusive care amid growing racism and anti-2SLGBTQ+ incidents.

  • Responding to Growing Community Challenges

    • Community Health Centres are adapting to increased demand caused by immigration, homelessness, mental health crises, and the drug poisoning crisis, ensuring care remains accessible for all.

  • Health and Wellness is part of all of our Programs and Services. 

We’re hosting a number of special events this month to highlight the important role that Community Health Centres have in keeping our communities healthy and our health system strong.

Join our activities this month and get a stamp on your Wellness Passport. Once your passport is filled, return it to us and you will get entered into a raffle for prize!

Activities include:

  • Attending a workshop
  • Get your blood pressure or blood sugar checked 
  • Complete our Health Equity Questionnaire
  • Join an Exercise Group
