Our Diabetes Education Program can help you stay well.

Our inter-professional team works with adults living with Type 2 diabetes and pre-diabetes. We can guide you on how to eat well, overcome stress and stay on track with your health goals. Our chiropodist can examine and treat any foot-related problems and injuries. We can also help connect you to more resources available in your community.

Diabetes Eye Screening Program

About two million people living with diabetes can experience vision loss.

Together with the Central Local Health Integrated Network (LHIN), we offer eye screening through the Ontario Telemedicine Network (OTN). Eye exams can be done at any OTN location throughout the city, but we can help you find one nearest you.

Your eyes can be sensitive after the appointment, so it’s a good idea to ask a friend or family member to drive you home.

Diabetes Community Health Promotion

We work together with community agencies and residents to support people living with diabetes. You can join our community discussions and workshops where you’ll learn about simple lifestyle changes that can help you take control of your health.

We welcome all patients – with and without a referral from your healthcare provider.

Diabetes Support Group

Meet other and share strategies and challenges with others who are living with or affected by diabetes.

Thursdays, 6:30 pm – 7:30 pm
